Abstract

In immunocompromised patients, Bacillus circulans is primarily regarded an opportunistic pathogen. This is a case report of septicaemia caused by Bacillus circulans in an extremely low birth weight preterm baby with respiratory distress. Although a laboratory contaminant but in immunocompromised host isolation of B. circulans should not be overlooked.

    1. Description of the case(s) The case reported is a novel case with the title describing the case. details of the case including demography, assessment, findings etc has been described. However comprehensive description of the management including drug dosage and duration is lacking. 2. Presentation of results Results are presented directly in a lucid manner, but the details like patient history, examination and investigation findings, differential diagnosis, management and outcome should be placed in chronological order. 3. How the style and organization of the paper communicates and represents key findings The simple style of writing and organized presentation does represents the findings of the case. 4. Literature analysis or discussion Literature analysis is done briefly with other cases reported represented in the tabular form, but limitations of the study is not presented. 5. Any other relevant comments Notable errors, other than grammatical ones include, typographical error of the references mentioned in the discussion of the study. Also use of the abbreviations should follow the elaborated description. Finally the description of sepsis and low birth weight categorization along with their references should be added.

    1. Description of the case(s) There is need to be categorized in several subsections. 2. Presentation of results Not bad. 3. How the style and organization of the paper communicates and represents key findings Very poor. 4. Literature analysis or discussion Need for substantially revision. 5. Any other relevant comments - Abstract should be substantially revised. it was brief contains no key message for readers, Thus, abstract should be carefully revised to be more interesting. - Keywords was missing!! - The sentence "It can also be a part of the body's normal micro flora in people who have been hospitalized for long time." need for citation. - In addition, the following sentences entitled "causing serious disease such as wound infection, abscess, meningitis, endophthalmitis, bacteremia, and even mortality owing to septicemia" also need for appropriate references. - Introduction was written with very low quality, the authors need to establish substantially revision on this section as well as highlighted the clinical relevance of this species in clinics. - There are some typos, grammatical, as well as punctuation mistakes, the manuscript need to Edited by a Native English Speaker. - Figure resolution was low, I have no seen any spore in figure 1B. - Usually spores can be created in condition with low nutrition rather than enrichment environment such as human blood, this sentence was confusing me !! - Do you have repeated blood cultures?, Was this species isolated from multiple replicate blood cultures? - This species was sensitive to all antibiotic agents, I wonder why it didn't respond to antibiotic treatment?? - In Table 1, the authors should provided more characteristics of the included cases such as risk factor, clinical presentation, diagnostic method, drug susceptibility testing results, therapeutic agents, treatment course duration, etc,. - In addition, the author also should discuss about relevant case reports that not be cited within the discussion, for example: 1. Infection of a cerebrospinal fluid shunt system by Bacillus circulans and Bacillus larvae. 2. Broviac catheter-related bacteraemias due to unusual pathogens in children with cancer: case reports with literature review. - Conclusion should be objective contains key clinical message.
