Childhood trauma is a major risk factor for internalizing disorders in adulthood, particularly depression and anxiety. Current evidence suggests that this relationship is partially mediated by cognitive and emotional processes that develop throughout life and influence vulnerability to psychopathology. This systematic review synthesized literature on cognitive mediators linking childhood trauma to internalizing disorders in adulthood. Searches were conducted in PubMed/MEDLINE, PsycINFO, Scopus, and Web of Science, covering studies published between 2000 and 2025. Twenty-five quantitative studies assessing mediation models were included. Results indicated that dysfunctional emotion regulation, rumination, and alexithymia were the most consistent mediators. Partial mediation was predominant, supporting the multifactorial nature of the trauma–psychopathology pathway.
