Abstract

Background Hepatic hemangiomas are common benign liver tumors, usually small and asymptomatic. Rarely, giant hemangiomas or diffuse hemangiomatosis can cause significant morbidity through mass effect. The simultaneous presentation of a giant cavernous hemangioma with diffuse hemangiomatosis is exceedingly uncommon. Case Presentation: We report the case of a 47-year-old female who presented with a seven-year history of a progressive abdominal mass and compressive symptoms. Cross-sectional imaging confirmed a giant hemangioma (21x10x29 cm) arising from the liver with diffuse hemangiomatosis. Management and Outcome: The patient underwent successful open surgical resection. Her postoperative course was uneventful, with complete symptomatic relief and discharge on the fifth postoperative day. Conclusion Surgical resection is a definitive treatment for symptomatic giant hemangiomas, even in the complex setting of diffuse hemangiomatosis. A history of oral contraceptive use may be a relevant associated factor.
