Eczematous External Otitis as an Allergic Rhinitis Multimorbidity and Therapeutic Effect of Sublingual Immunotherapy: A Case Report

Abstract

Background Eczematous external otitis is a rare but significant comorbidity of allergic rhinitis. While commonly treated with topical agents, systemic immunologic control is often overlooked. There is limited evidence regarding the effect of sublingual immunotherapy on this manifestation. Case Presentation: A 42-year-old woman with chronic bilateral ear itchiness and episodic serous discharge for 3 years, unresponsive to topical therapy. She also experienced symptoms of persistent moderate-to-severe allergic rhinitis. Skin prick test showed sensitization to house dust mite and dog dander. She was diagnosed with eczematous external otitis and allergic rhinitis. Sublingual Immunotherapy was initiated without additional topical or systemic therapy. Over 10 weeks, her nasal and ear symptoms improved significantly without the use of antihistamines or topical steroids. Conclusions This case highlights eczematous external otitis as a possible underrecognized comorbidity of allergic rhinitis and suggests that sublingual immunotherapy may play a therapeutic role.
