Background Milnacipran, widely prescribed for major depressive disorder and fibromyalgia, has a well-established adverse effect profile. Milnacipran-associated oral ulceration has never been reported to date. Case presentation A 54-year-old woman developed oral ulceration following milnacipran administration. This case highlights the importance of clinician awareness regarding rare adverse effects of common medications. Conclusions This represents the first documented case of milnacipran- associated oral ulceration. The report details the diagnostic and management approach for this unusual manifestation and discusses implications for milnacipran pharmacotherapy. Although rare, clinicians should consider this potential etiology. Given the retrospective nature of this report, the pathogenesis of milnacipran-associated ulceration remains incompletely understood and warrants further mechanistic investigation.
