Abstract

Background: Myocarditis is an inflammatory disease of the myocardium that may follow viral infections or surgical procedures, though the latter is rare. This case illustrates the unusual occurrence of acute myocarditis in a previously healthy 38-year-old man, three weeks after an uncomplicated vasectomy, with isolation of Corynebacterium kroppenstedtii , a rare opportunistic pathogen. Case Summary: A 38-year-old male presented with acute chest pain and elevated cardiac biomarkers. Cardiac imaging confirmed the diagnosis of acute myocarditis, and coronary angiography excluded coronary artery disease. Cardiac MRI revealed subepicardial late gadolinium enhancement. Blood cultures and a urethral swab both grew Corynebacterium kroppenstedtii . The patient responded to conservative treatment, and cardiac function normalized within a year. Conclusion: This case highlights a rare presentation of viral myocarditis likely triggered by a minor surgical procedure, with a possible iatrogenic introduction of an uncommon pathogen. The diagnostic role of multimodal imaging and careful microbiologic evaluation is emphasized.
