Background: International Medical Graduates (IMGs) form a growing segment of the UK healthcare workforce, with their numbers increasing by 121% since 2017. While their integration challenges are well-documented, their contributions to global health remain underexplored. This study examines the roles, barriers, and opportunities for IMGs practising in Scotland to engage in global health activities. Results: Quantitative data from an electronic survey (97 responses; 44% response rate) and qualitative insights from ten structured interviews revealed diverse global health contributions by IMGs. These include voluntary medical missions, capacity-building initiatives, research, collaborative partnerships, and advocacy efforts. However, barriers such as time constraints, limited awareness, financial challenges, and career pressures significantly limit their involvement. Identified facilitators include structural support mechanisms, such as protected time, dedicated funding, logistical aid, and recognition of global health activities within career progression frameworks. Conclusions: This study highlights the untapped potential of IMGs in advancing global health. Addressing systemic barriers and implementing supportive policies can empower IMGs to make impactful contributions, ultimately enhancing equitable healthcare development globally.
