Plasma Metagenomic Cell-Free DNA Sequencing Identifies Clinically Unsuspected Cases of Toxoplasma gondii Infection in Immunocompromised Patients: A Single Center Experience

Toxoplasmosis can be challenging to diagnose, particularly in immunocompromised patients with nonspecific clinical presentations. We present a retrospective, single-center case series of eight T. gondii infections identified by plasma metagenomic cell-free DNA (cfDNA) sequencing in allogeneic hematopoietic stem cell transplant recipients. Nested within a larger cohort evaluating cfDNA utilization, this case series highlights a positive clinical impact of cfDNA testing in 5/8 (62.5%) patients, including earlier diagnosis and changes in clinical management. These findings illustrate the potential utility of plasma cfDNA sequencing as a complement to conventional diagnostics across diverse clinical presentations of toxoplasmosis.
