Tuberculosis is a major public health problem worldwide. In Morocco, tuberculosis is still endemic, with almost 30,000 cases notified annually. The distribution of tuberculosis cases according to localization shows a significant percentage of extra- pulmonary tuberculosis (43%) compared with 57% for Pulmonary tuberculosis. Cutaneous tuberculosis is an exceptional extra-pulmonary form of the disease, with an incidence of 0.5% to 2% of cases. Given that the clinical symptoms of cutaneous tuberculosis are variable and nonspecific, diagnosis is not always easy. We report a rare case of cutaneous tuberculosis in a patient with a history of pulmonary and cutaneous sarcoidosis.

The case report was interesting but needs following clarifications - 1. A more detailed case description is required. 2. Language of the manuscript, specially the part on case description, needs to be modified. 3. Why was no histopathological evaluation of tissue obtained by skin biopsy done? 4. The account on sarcoidosis needs to be clarified. When was the patient diagnosed? Was he still on treatment for sarcoidosis when he developed cutaneous TB? Was the treatment for sarcoidosis continued with ATT? 5. Please recount other similar cases of cutaneous TB in literature.

Furthermore from a clinical perspective: - Please reconsider use of the term 'Koch's Bacillus' and use conventional nomenclature to refer to M. tuberculosis - Please clarify justification for discounting reactivation of pulmonary TB - the patient reported respiratory symptoms and has an abnormal CXR - what efforts were made to obtain microbiological samples - was a bronchoscopy or BAL undertaken? - Provide dosing of antimicrobials used - Provide information on past and current management for sarcoidosis (if any) Following significant revision of the submitted manuscript we would be happy to seek external peer-review.
