Abstract

Plasmodium vivax , one of the major species associated with human malaria, continues to be a major public health problem in many parts of the world. Numerous studies related to vivax malaria have described quantitative haematological findings (level of haemoglobin, thrombocytopaenia, haematocrit values), but diverse morphological changes of parasite forms within infected red blood cells (iRBCs) have been mentioned only in few studies. Here we report a case of a 13-year-old boy who presented with fever, significant low platelet counts and hypovolaemia that created a diagnostic dilemma. Detection of microgametocytes by microscopic examinations, further confirmed by multiplex nested PCR assays and response to anti-malarials, helped to make the diagnosis. We present an atypical case of vivax malaria with a review of morpho-variations of iRBCs and have summarized the characteristics that aid in creating increased awareness among laboratory health professionals and public health workers.

    This paper is presenting a case of vivax malaria infection with a challenge for the diagnosis. The vivax specie, predominant in the Asia region with a particular infection cycle can cause diagnosis issue particularly in severe case as it is unexpected. Red blood cells morphological variations are common future of plasmodium infection with a variety of presentation which is well presented in this article. Overall, the paper is quit well written but the English need to be reviewed before it can be published. Below are some comments/suggestions that need to be taking into account to improve the article Line 36: reduce the space after "among". Please consider this remark for the entire paper and correct as it is recurrent. Lines 65-66: please review the word "co-infection" and write I consistently Lines 74-75: please add year of publication to be consistent with the previous citations Line 91-92: please re-write the sentence to make it clearer to link to the topic or otherwise delete it as the current status doesn't link it to the topic Lines 102-105: same observation as above. The sentences need to be clarified or deleted Results: Line 110: you stated low blood pressure with measure of 110/40 mmHg. However, according to the patient age, the only low measure is the diastolic value. Please review Lines 111-125: please be consistent with the unit used for the platelet count measurement Line 133-134: the value of the "cardiac ejection fraction 55%" need to be more elaborated since there is no previous measurement to compare.
