Firstly it was not possible for us to correctly analyse the cost components in the OOP expenses as patients who sought care in public health facilities were not provided a hospital bill or expenditure statement. Secondly recall bias may have affected the retrospective assessment of the expenditures. Thirdly the approach of calculating the monthly household income based on total expenditure may have underestimated the actual household income. Notwithstanding these limitations, the study provides key information that will enable policy makers to prioritise interventions to ensure health equity and universal health coverage.
