Case Report: High-Resolution MRV Unmasks Hemodynamically Significant Venous Sinus Stenosis Missed by Conventional MRV in Idiopathic Intracranial Hypertension

Idiopathic intracranial hypertension (IIH) is characterized by elevated intracranial pressure of unknown etiology, with headache and visual disturbances being the predominant symptoms. Proposed pathophysiological mechanisms involve increased cerebrospinal fluid (CSF) production, impaired CSF reabsorption, hormonal influences, and elevated venous sinus pressure. This case report presents a 27-year-old female with a three-month history of headache and blurred vision, who was diagnosed with IIH and bilateral transverse-sigmoid sinus stenosis. It also highlights the limitations of conventional CE-MRV and the additional diagnostic value of high-resolution MRV in detecting clinically significant venous sinus stenosis.
