Background We examined the associations among oral contraceptive pill use, regular physical activity, and metabolic syndrome in female cancer survivors. Methods We used secondary data from the V to VIII (2010–2020) Korea National Health and Nutrition Examination Survey. This study was reported in accordance with the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines (https://www.strobe-statement.org/). Chi-square tests and multivariable logistic regression were used to analyze the data. Results The use of oral contraceptive pills was a significant risk factor for metabolic syndrome. Conversely, engaging in vigorous physical activity, walking, and muscle-strengthening exercises were significant protective factors against metabolic syndrome. Conclusions The use of oral contraceptives and engagement in physical activity influence female cancer survivors’ risk of developing metabolic syndrome. These findings support nurses and healthcare professionals in providing tailored contraceptive counseling and implementing physical activity–focused interventions to reduce the risk of metabolic syndrome among female cancer survivors.
