Abstract

Background Takotsubo cardiomyopathy (TCMP), also known as stress-induced cardiomyopathy, is a transient cardiac condition characterized by left ventricular dysfunction and apical ballooning. It primarily affects postmenopausal women and is often triggered by emotional or physical stress. However, TCMP can also occur in younger individuals, including postpartum women, where it may be mistaken for peripartum cardiomyopathy (PPCM). Distinguishing between TCMP and PPCM is crucial due to differences in prognosis and management strategies. Case Presentation: We present the case of a 34 years old Iranian woman who developed acute dyspnea, hypertension, and persistent headache five days postpartum following her first cesarean section. The first diagnosis impression was PPCM, but further imaging, including echocardiography and cardiac magnetic resonance (CMR), revealed findings characteristic of TCMP, such as mid to apical left ventricular akinesia with preserved basal function. She received medical therapy for heart failure, leading to a gradual symptomatic and functional recovery. Conclusions Due to the clinical overlap of TCMP with PPCM, early imaging and biochemical tests are necessary for accurate diagnosis. Early recognition and appropriate management of TCMP can lead to complete recovery of heart function.
