The increasing adoption of the Medical Internet of Things (MIoT) in healthcare has enabled end users to collect and analyze sensitive patient data through various devices. However, this capability has raised significant privacy concerns, as MIoT devices often handle highly confidential information. The processes involved in data sharing can expose healthcare data to security and privacy risks. To mitigate these challenges, integrating MIoT with blockchain technology has emerged as a promising solution for ensuring secure transactions in healthcare settings. However, the inherent complexity of blockchain, characterized by intricate mathematical proofs and consensus mechanisms, demands substantial computational resources, making direct integration with IoT devices less feasible. In response to these challenges, we present a smart blockchain-driven framework (SBCDF) specifically designed for MIoT applications in healthcare. Our proposed SBCDF features two key mechanisms: first level privacy, incorporating service oriented layer approach inside the blockchain and second level privacy, presenting an anomaly based intrusion detection system. This two-level privacy scheme first incorporates a blockchain module to securely register and authenticate MIoT devices followed by transaction contract allowing MIoT devices to transit their data. Additionally, we employ an anomaly based intrusion detection system (IDS) in the second level of privacy to train and evaluate the overall proposed privacy scheme. This integration of threat intelligence detection with blockchain creates an ecosystem where threats can be monitored and addressed in real-time, ensuring that MIoT networks remain resilient against evolving cyber threats. Experimental results demonstrate that our proposed SBCDF framework comprehensively safeguards the privacy of MIoT data while maintaining the vital attributes of security and transparency that are inherent to blockchain-based systems, highlighting its potential to be implemented in healthcare MIoT applications.
