Objective This study aims to explore the cognitive perceptions, psychological state, and behavioral changes in women infected with high-risk human papillomavirus (HR-HPV), providing a theoretical and practical foundation for clinical psychological interventions. Methods Using purposive sampling, 34 female patients with HR-HPV who attended the Gynecological Outpatient Clinic at Shanghai General Hospital between July and September 2023 were selected as participants. Semi-structured interviews and Colaizzi's phenomenological analysis, grounded in the cognitive-affective-behavioral theory, were used to collect and analyze data on patients' background information, psychological state, coping strategies, interpersonal relationship dynamics, and self-perception. The clinical trial registration has been completed and Chinese clinical trial registration Number is ChiCTR2400081683. The clinical trial was registered on 8 March 2024 by the Shanghai First People's Hospital. Results Most participants demonstrated limited awareness of HR-HPV and experienced emotions such as confusion, shock, anxiety, and concern following their diagnosis. Psychological and behavioral aspects related to sexuality and social interaction were notably affected, including feelings of stigma, deterioration in sexual relationships, diminished self-esteem and confidence, and reduced social support, which hindered their ability to cope with the condition. Conclusion Women diagnosed with HR-HPV face considerable psychological and behavioral challenges after learning of their infection. Healthcare professionals should provide accurate information about the disease and offer psychological support to help patients address the emotional and mental health issues that arise from the infection.
