Background : Pain management during dental procedures, especially vital pulp therapy, is crucial in pediatric dentistry. This study aimed to evaluate the effect of pre-medication with ibuprofen on pain during vital pulp therapy in children with molar-incisor hypomineralization (MIH). Methods: In this randomized clinical trial, 32 children aged 6–9 years with MIH were allocated to an intervention group (n=16) or a control group (n=16). The intervention group received ibuprofen 45 minutes before treatment; the control group received a placebo. Pain was assessed before treatment and at pulp entry using the Five-Face Visual Analog Scale (VAS) and during the procedure using the Sound, Eye, Motor (SEM) scale. Data were analyzed using SPSS 26 at a significance level of 0.05. Results: Nineteen participants (59.4%) were boys and 13 (40.6%) were girls. Mean ± SD age was 7.99 ± 0.59 years in the intervention group and 8.01 ± 0.47 years in the control group (p=0.221). The intervention group reported significantly lower pain scores than the control group on both VAS (p=0.004) and SEM (p=0.001) scales. Conclusion: Pre-medication with ibuprofen suspension significantly reduces pain during vital pulp therapy in mandibular first permanent molars affected by MIH in children aged 6–9 years.
