Background Systemic lupus erythematosus is a chronic, multi-organ, systemic autoimmune disorder that is prevalent in women and can affect those of reproductive age. This study aimed to explore the latent profile characteristics of reproductive concern in women of childbearing age with systemic lupus erythematosus, and analyze the differences among different categories of patients. Methods A questionnaire survey was administered to 332 female patients with systemic lupus erythematosus of childbearing age in four tertiary grade A general hospitals in Mianyang City, China. We utilized a general information questionnaire, Reproductive Concerns After Cancer Scale (RCAC), Medical Coping Modes Questionnaire (MCMQ), and Social Support Revaluated Scale (SSRS). Latent profile analysis and multiple logistic regression models were used to explore latent profile characteristics and factors influencing reproductive concerns. Results Four latent profile groups were obtained: low reproductive concern–high infertility acceptance (12.66%), moderate reproductive concern–concern about personal health (18.95%), moderate reproductive concern–concern about children’s health (45.64%), and high reproductive concern–balanced (22.75%). Factors influencing the reproductive concern categories included age, marital status, annual income, method of medical payment, number of children, fertility intention, history of adverse pregnancy outcomes, disease activity, social support, and medical coping strategies. Conclusions Reproductive concerns in women of childbearing age with systemic lupus erythematosus exhibited significant heterogeneity. In clinical nursing, personalized intervention measures should be developed based on different categorical characteristics and influencing factors to reduce reproductive concern levels in patients.
