Background Pranic Healing, a naturopathic biofield therapy, enhances well-being by harmonizing life energy (Prāṇa). Research supports its effectiveness in reducing depression, anxiety, increase quality of sleep and functional health. Objective This randomized controlled trial evaluated the effects of Mass Pranic Healing (MPH) on lung function, physical performance, and blood pressure compared to Sham Pranic Healing (SPH). It also explored the interaction between mindfulness and treatments on these outcomes and the difference in experiences were evaluated. Method A four-week trial involved 78 paramedical students (mean age 18.86 years), randomly assigned to MPH or SPH groups. Lung function, physical performance, blood pressure, and mindfulness were assessed before and after the intervention. Statistical analyses included Linear Mixed Model analysis and repeated measures ANOVA. Experiences of participants have been Thematically analysed and difference in experiences are reported. Findings: SPH participants experienced a greater decline in pulmonary function, while mindfulness moderated lung function changes. In the MPH group, high mindfulness improved FEV1 (+ 70 mL, 95% CI: 2.169–2.516 to 2.231–2.579), whereas SPH showed a decline (− 23 mL, 95% CI: 2.474–2.881 to 2.269–2.632). MPH significantly enhanced physical performance, whereas SPH showed no significant changes. Sensations such as tingling and the presence of at least one Pranic Energy Sensation correlated positively with mindfulness. Qualitative feedback revealed, MPH participants had enhanced physical and psychological health. Conclusion MPH along with higher mindfulness, led to improved lung function and physical endurance. The synergistic potential of energy-based therapies and mindfulness in supporting respiratory health, physical vitality and wellbeing.
