Abstract

This case report presents a 66-year-old woman recently emigrated from Peru to Spain, who was diagnosed with a stress fracture of the left tibia. Radiological fi ndings revealed a saber-shaped angulation of both tibia and fi bula, suggesting an underlying metabolic bone disease. Laboratory tests showed elevated alkaline phosphatase and low phosphorus levels. The diagnostic process considered Paget’s disease, osteomalacia, hyperparathyroidism, and phosphorus defi ciency. This case emphasizes the importance of early diagnosis and a multidisciplinary approach in managing atypical stress fractures in older adults.
