Abstract

Stanford type A aortic dissection (TAAD) with concomitant abdominal aortic dissection aneurysm presents a complex clinical challenge and has traditionally required open surgical repair [1]. Notably, concomitant abdominal aortic aneurysmal degeneration further complicates treatment paradigms, particularly when critical branches (e.g., renal arteries) originate from the false lumen—a configuration associated with higher rates of target vessel instability and reintervention. Current guidelines emphasize intervention for complicated dissections with refractory symptoms or aneurysmal expansion >5.5–6.0 cm despite optimal medical therapy [2]. Endovascular solutions, such as fenestrated-branched endografts, offer a less invasive alternative for high-risk patients. This report details a single-stage total endovascular reconstruction for TAAD with abdominal aneurysmal degeneration, leveraging precise fenestration alignment and covered stents to address multi-segmental disease while mitigating SCI and endoleak risks.
