Intracranial Dural Extramedullary Hematopoiesis in Myelofibrosis: MRI and CT Imaging Features from a Rare Case Report

Intracranial extramedullary hematopoiesis (EMH) is a rare clinical condition. This article reports a case of a 54-year-old male patient with a 6-year history of myelofibrosis, who had been relying on blood transfusion support therapy and cell depletion therapy for a long time. Due to persistent headache and dizziness, a neuroimaging examination (CT/MRI) revealed diffuse thickening of the dura mater with the formation of a huge soft tissue mass (maximum diameter > 10 cm). Combined with the imaging features (T2WI showing a significantly low signal, suggesting hemosiderin deposition) and pathological analysis, common lesions such as meningioma were excluded. Eventually, the diagnosis of intracranial EMH was made. This case suggests that for patients with myelofibrosis and neurological symptoms, EMH should be considered. The typical low signal on T2WI has important diagnostic value.
