We present the case of a 73-year-old male with a history of situs inversus totalis who presented with hypoxemic respiratory failure and cardiac dysfunction. The patient’s complex anatomy posed unique diagnostic and therapeutic challenges. Diagnostic methods, including chest computed tomography (CT), echocardiography, and pulmonary function tests, were employed to evaluate the interplay between his congenital anomaly, respiratory failure, and cardiac impairment. This case highlights the importance of tailored diagnostic approaches in patients with situs inversus and underscores the need for multidisciplinary management. We discuss the diagnostic modalities used, their findings, and the clinical implications of managing such cases.
