A new technique of scleral suture fixation for posterior chamber lenses

We present a novel scleral suture fixation technique for repositioning a dislocated intraocular lens (IOL). Methods: A 1 mL needle is used to guide and extract the double-armed 10-0 straight polypropylene suture through the paracentesis, positioned 2 mm from the limbus at the 3 and 9 o’clock meridians. The suture is then secured around the haptic, forming a specialized knot. The same technique is applied to the other haptic. Both sutures are adjusted and anchored within scleral grooves to conceal the knots. Results: No complications were noted, and follow-up assessments showed an uncorrected visual acuity of 0.8 (decimal). Conclusions: Considering axial, sagittal, and rotational stability principles, this scleral fixation approach provides a viable alternative for stabilizing a dislocated three-piece IOL.
