Brief Communication: Mid to Long-term Outcomes of Metabolic Surgery in Remission of Type-2 Diabetes Mellitus in Patients with BMI 25-29 Kg/m2

Introduction While there are published studies that demonstrate the efficacy of MBS in overweight patients with a BMI of less than 30 kg/m², particularly in terms of remission and improvement of T2DM, these studies typically have follow-up periods of less than three years and do not address the recurrence of T2DM. This study aimed to assess the rates of remission and recurrence of T2DM during mid to long-term follow-ups in patients with a BMI of less than 30 following MBS. Material and Methods The retrospective cohort study was designed on patients aged between 18–65 years old with T2DM and BMI < 30 kg/m 2 who underwent MBS in Rasoul-e Akram Hospital, an IFSO-EC endorsed academic and tertiary center for metabolic and bariatric surgery from March 2016 to August 2019. Results A total of 18 patients with T2DM who underwent MBS were enrolled in this study. Preoperative mean BMI was 28.5 ± 0.9 kg/m 2 . OAGB was performed in 12 (66.7%) and RYGB in 6 (33.3%). 11 patients (61.1%) were on insulin and 12 patients (66.6%) were on oral anti-glycemic agents. The mean number of the oral anti-glycemic agents was 1.31 ± 1.0. The mean duration time of T2DM was 10.0 ± 8.0 years. Conclusion The mid to long-term outcomes of MBS in patients with a BMI of 25–29 kg/m² indicate that these procedures can effectively induce remission of T2DM and improve overall health.. The recurrence of T2DM in overweight patients should be considered higher compared to the patients with obesity in long-term follow-ups.
